6. Red flags — escalate / transfer

🚩 Red flags

Recurrent stroke — any new focal deficit or a positive BE-FAST — activate the stroke/transfer pathway; document time last-known-well (time-critical).

Aspiration event / pneumonia — coughing/choking with color change, congestion, fever, or hypoxia — assess, position, suction, and escalate (route to Taste/Swallowing/Respiratory).

DVT / PE — unilateral calf swelling/pain, or sudden dyspnea/hypoxia — escalate/transfer (time-critical).

Major bleeding on antithrombotic therapy or a post-fall head strike on anticoagulation — activate the appropriate pathway (route to Anticoagulation).
