Deliver and evaluate rehabilitation-nursing carryover across the deficit set and coordinate therapy (route program detail to Restorative).
Manage secondary prevention (BP goal, antithrombotic with bleeding surveillance, statin, glucose) and teach it toward discharge.
Provide complication surveillance (aspiration, DVT, mood, recurrent stroke) with immediate escalation of a new deficit.
Teaching / training (to mastery)
Teach the resident/caregiver the secondary-prevention plan (BP, medications, lifestyle), the deficit-specific home strategies, and the stroke warning signs (BE-FAST), with return-demonstration/teach-back.
Train CNAs on deficit-change reporting (escalate a new deficit immediately), aspiration and DVT precautions, and communication strategies, with documented understanding.
