5. Skilled interventions and teaching

Skilled interventions

  • Deliver and evaluate rehabilitation-nursing carryover across the deficit set and coordinate therapy (route program detail to Restorative).
  • Manage secondary prevention (BP goal, antithrombotic with bleeding surveillance, statin, glucose) and teach it toward discharge.
  • Provide complication surveillance (aspiration, DVT, mood, recurrent stroke) with immediate escalation of a new deficit.

Teaching / training (to mastery)

  • Teach the resident/caregiver the secondary-prevention plan (BP, medications, lifestyle), the deficit-specific home strategies, and the stroke warning signs (BE-FAST), with return-demonstration/teach-back.
  • Train CNAs on deficit-change reporting (escalate a new deficit immediately), aspiration and DVT precautions, and communication strategies, with documented understanding.
