4. Abnormal findings — the necessity chain

Finding → severity → comparison → risk → intervention → notification (provider + representative) → orders → response → reassessment.

Model abnormal 1

Post-operative eye complication: the operative eye now with increasing pain, worsening redness, new purulent discharge, and a reported decrease in vision — concerning for endophthalmitis, a sight-threatening emergency. The findings assessed, the ophthalmology service and the provider notified urgently, the drop regimen and activity clarified, and the eye protected; the resident monitored. Will reassess; supports the Section B/I update. Skilled recognition required, as a post-operative eye infection is a vision emergency.

Model abnormal 2

Acute vision change: a sudden loss of vision (or a new visual-field cut) in a resident — concerning for a retinal event, acute glaucoma, or a stroke. Assessed for associated signs (eye pain, halos, nausea suggesting acute angle-closure; a focal neurologic deficit suggesting stroke), the provider notified urgently, and the appropriate pathway activated (ophthalmology or the stroke pathway); the responsible party notified. Will reassess; supports the Section B/I update. Skilled recognition required, as sudden vision loss is time-critical. (A stroke presentation routes to the Neurological reference.)
