Strong — defensible

“Heart regular. No edema. Weight stable.”

No values, no comparison, no parameter, no skilled rationale.

The HF-recovery model note in §3: weight vs prior/target, edema grade, lung and rhythm findings, and the correlation with the diuretic/sodium order.

“Lasix given.”

No weight/edema trigger, no response, no MDS support.

“4-lb gain and new 3+ edema → parameter-based diuretic per order → relief after voiding; provider notified; reassess next shift.”

“Irregular pulse noted.”

No rate, no BP, no symptoms, no action.

The arrhythmia model note in §4: apical/radial rate, deficit, BP, symptoms, held dose, SBAR, ECG/rate-control orders.
