5. Skilled interventions and teaching

Skilled interventions

  • Obtain and trend daily weights; apply parameter-based diuretic/cardiac orders and document the response.
  • Provide skilled observation of the unstable cardiac status with parameter-based escalation; administer and monitor IV/oral cardiac medications and anticoagulants.
  • Correlate volume findings (weight, edema, lungs) with the plan and coordinate labs/telemetry as ordered.

Teaching / training (to mastery)

  • Teach the resident/representative daily-weight monitoring, the sodium/fluid plan, medication purpose and adherence, and the warning signs that prompt a call, with teach-back.
  • Train CNAs on accurate daily weights, edema/intake reporting, and orthostatic-safety measures, with documented understanding.
