Strong — defensible

“Slept well. CPAP on.”

No settings, no tolerance, no desaturation, no participation link.

The CPAP model note in §3: the ordered pressure and mask seal, tolerance and adherence, the overnight SpO₂/apnea observation, and the reason skilled device management was required.

“Up all night.”

No pattern comparison, no delirium/trigger screen, no action.

The sleep-wake-reversal model note in §4: the change versus the pattern, the CAM/trigger screen, notification, and sleep-hygiene/safety measures.

“Restless, sleeping poorly.”

No cause, no impact, no action.

Document the contributing factor (pain/dyspnea), the impact on participation, and the intervention/response.
