Neurological conditions are common qualifying Part A stays: post-stroke rehabilitation, new or fluctuating focal deficits, seizure disorders, post-neurosurgical recovery, and progressive neuromuscular decline. The skilled element is the neurological assessment itself — detecting a new or evolving deficit, monitoring for the complications of a recent stroke, maintaining seizure precautions, and carrying over the rehabilitation plan — because a change may be time-critical and a layperson cannot recognize it.
The purpose of this module is to prove, for each billed day, that skilled neurological observation, rehabilitation nursing, teaching, or management of the plan was reasonable, necessary, needed daily, and available only in a SNF under Pub. 100-02 Chapter 8. The neuro exam is the same as in home care; the coverage spine is re-pointed and the Section GG / cognition linkage is added.
