Skilled interventions

  • Perform the focused neurological/deficit exam each shift and compare to the baseline; escalate any new or evolving deficit immediately.
  • Maintain seizure precautions and administer/monitor anticonvulsants; reinforce aspiration precautions after a stroke.
  • Deliver and evaluate stroke-rehabilitation-nursing carryover and coordinate therapy (route program detail to Restorative).

Teaching / training (to mastery)

  • Teach the resident/representative the stroke warning signs (BE-FAST), seizure safety, aspiration precautions, medication purpose, and safe mobility, with return-demonstration/teach-back.
  • Train CNAs on deficit-change reporting, communication techniques for aphasia, aspiration precautions, and safe transfer, with documented understanding.
