MDS & care-plan linkage The ability to make self understood (B0700) and to understand others (B0800) are coded from the record, with cognition in Section C; the note must support each (F641). Each note ties to the communication care-plan problem/goal (baseline F655 → comprehensive F656 → revision F657); a new deficit may trigger a Significant Change assessment (F637).
