Daily Skilled Note & Survey-Readiness Checklist

The pre-flight check run before signing a Part A skilled note — the daily-skilled requirements, skilled-service justification, MDS-supportive documentation, care-plan currency, teaching, care coordination and change-of-condition notification, safety and closeout, and authentication items that map to the most-cited SNF coverage denials and survey deficiencies.

Scope

The visit-and-day-level requirements for a Medicare Part A skilled-nursing note, plus the RoP items surveyors examine, organized as pass/fail blocks. A note that clears every block carries its own defense on both an ADR coverage review and a survey.

How to use this module

Run this checklist before signing every Part A skilled note, and use the survey blocks as a quarterly Triple-Check and mock-survey tool. It operationalizes the Foundations disciplines (necessity sentence, daily-skilled framing, quantified outcomes, teach-back, closed-loop coordination, MDS support, care-plan linkage) and the self-contained rule — every value is stated so a covering nurse who has never met the resident can act on it. Where an item names a threshold, confirm it against current orders and the care plan. Treat any unchecked item as a documentation gap to close before authentication, not after a determination or a survey.

How the blocks map to the two frames

Blocks 1–2 establish that the day occurred, the resident qualifies, and a skilled service was required today (coverage).

Block 3 proves the MDS is supported by the record (payment + accuracy, F641).

Block 4 proves the care plan is current and linked (F655/F656/F657).

Block 5 proves teaching produced measured learning.

Block 6 proves coordination and change-of-condition notification closed the loop (F580).

Blocks 7–8 establish resident safety at closeout and a valid, contemporaneous signature.
