6. Care coordination & change-of-condition notification

• Provider contact logged with date/time, method, and content (SBAR or a specific request with the relevant data).
• Resident representative notified on any change of condition (F580) — with date/time.
• Response received, or a specific follow-up time set if pending.
• Other-discipline coordination noted (therapy, dietitian, social work, pharmacy).
• If the change is significant and non-self-limiting, a Significant Change assessment initiated (F637); care plan revised.
