How to use this module
This module supplies psychiatric and mental-health assessment and the skilled language that proves it required a licensed nurse — validated mood and anxiety screening, risk screening with an immediate safety response, recognition of psychiatric symptoms, and connection to care. Cognition and delirium are assessed in Cognitive / Mental Status and psychosocial stressors in Psychosocial / Caregiver; an acute behavioral change is always evaluated for a medical or delirium cause. When any risk to self or others is identified, the priority is immediate safety and escalation, including crisis resources (the 988 Suicide and Crisis Lifeline) and emergency services. Each narrative is a structured example to be edited to the actual visit.
