4. Normal findings

The reference range below is the yardstick for “normal/at-baseline.” Normal findings are documented in Mode A with the skilled rationale that follows in Section 6 — never as “WNL” alone.

  • Mood and affect at the patient's baseline; a negative or low depression screen.
  • Anxiety at baseline; a negative or low anxiety screen.
  • Denies thoughts of self-harm and of harm to others.
  • No psychotic or manic symptoms and no severe agitation.
  • A psychiatric condition, if present, stable on treatment with adherence and tolerable side effects.
  • No substance-use concern, or a stable, managed status.
  • Function, engagement, and safety at baseline.
