10. Response and reassessment

Every intervention and teaching action requires a documented response that closes the loop. An intervention without a reassessment is an audit gap.

• The effect of an intervention — for example a change in the PHQ-9 or GAD-7, engagement with a referral, or the outcome of a safety response.
• Treatment adherence and side-effect tolerability after reinforcement.
• Teaching effectiveness: teach-back of the treatment plan, coping strategies, and crisis resources.
• Whether symptoms or risk have changed and what that indicates for the plan.
• The plan for reassessment timing and what will be re-evaluated next visit.
