• Signs of internal preoccupation, responding to internal stimuli, or disorganized thought (psychosis).
• Grooming and self-care as they reflect mood (detail in General / Constitutional).
• Any acute behavioral change suggesting a medical or delirium cause (assessed with Cognitive / Mental Status).
