• Dizziness or unsteadiness, and the situations that provoke it.
• The medication routine — how medications are organized, taken, and stored, and any confusion.
• The ability to call for help and what the plan is in an emergency.
• Safety concerns the patient or caregiver raises about the home or the situation.
