4. Normal findings

The reference range below is the yardstick for “normal/at-baseline.” Normal findings are documented in Mode A with the skilled rationale that follows in Section 6 — never as “WNL” alone.

  • Low fall risk, or an elevated risk mitigated with measures in place and used correctly.
  • The home environment free of significant hazards, or hazards addressed.
  • Medications stored safely, organized, and understood, with no high-risk errors or duplications.
  • Emergency preparedness adequate — a working phone, current contacts, functioning detectors, and the ability to summon help.
  • No wandering or elopement concern.
  • No indicators of abuse, neglect, or self-neglect.
