Document the skilled interventions delivered this visit; each should connect to a finding and carry a reassessment in Section 10.

  • Fall-risk assessment with specific mitigation — recommending grab bars, removing throw rugs, improving lighting, and addressing footwear and orthostatic contributors.
  • Home-hazard identification and coordination of correction.
  • Medication reconciliation with setup of safe storage and an organization system, and flagging of high-risk or duplicated medications to the provider.
  • Emergency-preparedness setup — detectors, current contacts, a plan to summon help, oxygen safety, and an equipment backup plan.
  • Coordination of durable medical equipment, physical or occupational therapy, or social work, and mandated reporting of suspected abuse or neglect per state law and agency policy.
