Diabetes Mellitus
Diabetes mellitus overlay. Supplements the Endocrine / Metabolic assessment with the individualized glycemic targets, insulin and monitoring management, hypoglycemia and decompensation surveillance, and sick-day and foot-protection teaching specific to diabetes. It does not replace the Endocrine / Metabolic module — the general endocrine assessment lives there; this overlay adds the disease-specific parameters, monitoring, and escalation. Targets are individualized; this overlay never assumes one target for every patient.

How to use this overlay Apply this overlay whenever diabetes is on the plan of care, alongside the Endocrine / Metabolic assessment. Section 2 states this patient’s individualized ordered glycemic targets and notify thresholds in self-contained terms so a nurse covering the patient can act on them without prior knowledge of the patient — always confirmed against the current orders. Every scenario states the actual glucose values and their objective reference (the ordered individualized range, a measured value, the sick-day rule), never “at baseline.” Pin the clinical detail to the current ADA Standards of Care. Each phrase and scenario is a scaffold to complete with the visit’s actual values — not a phrase to copy. |
