3. Plan-of-care goals

The overlay goals, tied to the ordered parameters above, are:

  • Glucose is maintained within the patient’s individualized ordered range, without severe hypoglycemic or hyperglycemic events.
  • Insulin, injectable- or oral-agent, and monitoring technique are correct on observation, so the regimen is actually delivered.
  • The patient and caregiver recognize and treat hypoglycemia — including glucagon where ordered — and follow the sick-day rules.
  • The feet are protected, with skin integrity and protective-sensation status checked each visit and any change reported.
  • Glycemic control improves toward the individualized target over the episode of care, with the A1c trend documented, and complications and readmissions decrease.
