5. Medication and treatment monitoring
Monitor the diabetes regimen for delivery, effect, and adverse effects, correlating with the ordered individualized targets:
- Insulin — basal and bolus or a correction scale — for dose accuracy against the order, injection or pump technique, site rotation, and storage, and for hypoglycemia as the principal adverse effect.
- Oral and non-insulin injectable agents for adherence, effect, and agent-specific effects (for example gastrointestinal effects or the volume-status and genitourinary considerations of certain agents) per the current standards.
- A correction or sliding scale where ordered — applied to the measured glucose within the ordered parameters.
- Steroid-induced hyperglycemia where the patient is on corticosteroids — increased monitoring and coordination (cross-reference COPD overlay).
- The monitoring device — meter calibration or continuous-glucose-monitor function, alerts, and the data trend.
- The A1c trend against the individualized target as the measure of overall control.
- Vaccine and complication-screening status (eye, renal, foot) as diabetes-care measures per the current standards.
