Diabetes is among the most common reasons patients receive skilled home health and a frequent source of emergencies — severe hypoglycemia, diabetic ketoacidosis, the hyperosmolar hyperglycemic state — and of the complications that drive readmission. The skilled value of this overlay is interpreting the glucose pattern against the patient’s individualized ordered targets, ensuring insulin and monitoring technique actually work, recognizing decompensation early, and protecting the feet, all documented so any nurse can act on it.
This overlay supplements the Endocrine / Metabolic assessment. The general endocrine exam lives in that module; here the clinician adds the individualized glycemic targets, insulin and monitoring management, hypoglycemia and decompensation surveillance, the diabetic-foot link, and sick-day teaching. Because glycemic targets are individualized, the ordered range for this patient — not a generic number — governs every interpretation.
