3. Key visit elements

Add these hypertension-specific elements at each visit alongside the cardiovascular and vital-signs examinations — this is not a repeat of the full exam:

  • Blood pressure measured with correct technique (cuff size, 5-minute rest, seated with the arm supported), repeated if elevated, and compared to the ordered parameters and the home log.
  • Home blood-pressure log reviewed: readings, technique, adherence, and the average against the ordered parameters.
  • Symptom screen for end-organ involvement: headache, visual change, chest pain, dyspnea, and focal weakness or speech change.
  • Orthostatic check if the patient is symptomatic or on agents prone to orthostasis.
  • Medication adherence and adverse effects; screen for NSAIDs, decongestants, and other agents that raise blood pressure.
