🚩 Red flags — escalate
Blood pressure above the ordered threshold without symptoms — recheck with correct technique after rest; provider notification per parameters; review adherence and OTC use.
Blood pressure above 180/120 mm Hg WITH symptoms such as severe headache, visual change, chest pain, dyspnea, or focal weakness or speech change — possible hypertensive emergency: activate emergency response (911) and notify the provider per policy.
Symptomatic hypotension or over-treatment, or syncope — provider notification per parameters; fall precautions.
New focal neurologic symptoms (face droop, arm weakness, speech change) — activate emergency response (911) for possible stroke.
