Document each teaching point with topic, method, audience, and measured mastery (teach-back percentage or return demonstration), per Foundations.

  • Home blood-pressure technique — cuff size and placement, rest, timing, and recording.
  • The report thresholds and whom to call; when symptoms make an elevation an emergency (see red flags).
  • A sodium-restricted, DASH-style dietary pattern and label reading; weight and activity.
  • Medication adherence, no abrupt discontinuation, adverse effects, and avoiding NSAIDs and OTC stimulants.
