Daily-note bank — change-of-condition / at-risk days
Scenario
Model daily note (edit to the shift)
Anaphylaxis
New hives, facial/lip swelling, throat tightness, wheeze after an exposure — epinephrine per protocol without delay, exposure stopped, airway supported, EMS/transfer activated, provider/representative notified, response monitored (2nd dose per protocol); emergency; supports Section O/I. (Respiratory.)
Immunosuppressed infection
Low-grade fever/subtle change on immunosuppression — provider notified promptly, cultures/labs/evaluation per order, empiric management considered, close sepsis monitoring, representative notified; reassess; supports Section O/I. (Sepsis overlay.)
Severe drug reaction
Spreading rash with mucosal involvement/blistering and fever — suspected drug stopped, provider notified urgently, monitored/transferred per severity (SJS/TEN concern); documented; supports Section I.
Linked disease-specific scenarios
Airway compromise from anaphylaxis/angioedema routes to the Respiratory reference; cytopenias and neutropenic fever route to the Hematologic reference and the Sepsis overlay; IVIG/immunotherapy access routes to the Lines/Drains/Devices reference; immunosuppressed infection routes to the Sepsis overlay.
