Unplanned weight loss and dehydration are sentinel indicators of nursing-home care and frequent citations. The facility must ensure each resident maintains acceptable nutritional parameters and is offered sufficient fluid, unless clinically contraindicated (F692), and that enteral feeding is managed to prevent complications (F693).
The documentation must show weight monitoring against the significant-loss thresholds, intake and hydration tracking, the therapeutic/altered diet and its rationale, dysphagia recognition and aspiration precautions, and skilled management of any decline or feeding complication.
