MDS & care-plan linkage Weight loss/gain (K0300/K0310) and nutritional approaches (K0510) are coded in Section K; the record must document weights, the calculation, and the approach so the MDS is supported (F641). Each note ties to the nutrition care-plan problem/goal (baseline F655 → comprehensive F656 → revision F657); a significant weight change prompts a care-plan revision and may trigger a Significant Change assessment (F637).
