Skilled interventions
- Monitor and record weights on schedule and calculate the loss/gain against the thresholds; escalate a significant change.
- Implement the therapeutic/altered diet and aspiration precautions; assist and cue at meals as needed; track intake and hydration.
- Manage enteral feeding per order (rate, residual, flushes, site care) and recognize intolerance early.
Teaching / training (to mastery)
- Teach the resident/representative the diet rationale, aspiration precautions, and enteral-feeding care, with teach-back.
- Train CNAs on safe feeding technique, aspiration precautions, and intake documentation, with return-demonstration.
