2. What to assess, technique, and required data

Track weight, intake, swallowing, and feeding tolerance:

  • Weight trend against the significant-loss thresholds: about 5% in 30 days, 7.5% in 90 days, or 10% in 180 days (unplanned).
  • Percent meal intake and hydration status; the diet order and its texture/consistency (using IDDSI levels where adopted).
  • Dysphagia signs — coughing, choking, pocketing, wet/gurgly voice, prolonged meals — and aspiration precautions; for enteral feeds, tolerance, residual per policy, and site care.

MDS-supportive data

Weight loss/gain is coded in MDS Section K (K0300/K0310) and nutritional approaches in K0510 (parenteral/IV, feeding tube, mechanically altered/therapeutic diet); the record must document weights, the loss/gain calculation, and the approach so Section K is supported (F641).
