Percent meal intake and hydration status; the diet order and its texture/consistency (using IDDSI levels where adopted).
Dysphagia signs — coughing, choking, pocketing, wet/gurgly voice, prolonged meals — and aspiration precautions; for enteral feeds, tolerance, residual per policy, and site care.
