MDS & care-plan linkage Restorative techniques are coded in Section O0500 when provided 15+ minutes/day; the record must document technique, minutes, and days so the MDS is supported (F641). Function is coded in Section GG. Each note ties to the functional/restorative care-plan goal (baseline F655 → comprehensive F656 → revision F657); a functional decline or a met goal prompts a care-plan revision.
