New severe room-spinning vertigo with nausea and unsteadiness — fall precautions intensified, central-cause screen, provider notified, vestibular measures/workup ordered, safe mobility managed, representative notified; reassess; supports the ear/fall goal. (Falls/Neuro.)

Sudden hearing loss

Sudden unexplained hearing loss in one ear — provider notified urgently (treatment-time-sensitive), evaluation ordered, communication strategies adjusted; documented; supports Section B/I.
