Why it fails
Strong — defensible
“Arm swollen. Compression on.”
No measurement, no skin/infection surveillance, no limb protection, no rationale.
The lymphedema model note in §3: the girth versus prior, correct compression with limb protection, the skin/cellulitis surveillance, and the reason skilled care was required.
“Leg red.”
In a lymphedematous limb this may be cellulitis/sepsis — no escalation.

“Compression applied.”
No tolerance, no perfusion check, no effect.
“Ordered compression applied correctly, limb protected, tolerated; girth improved versus prior; skin intact.”
