Post-hysterectomy care

Day 3: minimal expected spotting, abdominal incision clean/dry with staples and no erythema, no fever, tolerating diet with bowel sounds, VTE prophylaxis given, activity per order. Skilled hemorrhage/dehiscence/infection/VTE surveillance required; supports Section I. (Anticoag overlay.)

Pessary management

Pessary in place with a comfortable fit, vaginal/perineal skin intact without erosion/discharge, cleaned per schedule. Skilled pessary assessment and care required; supports Section I.

Post-prolapse-repair care

Perineal/vaginal repair site clean, minimal expected discharge, activity/straining restrictions observed, no complication signs. Skilled post-operative surveillance required; supports Section I.
