Vaginal-cuff dehiscence / evisceration after hysterectomy — a sudden gush, heavy bleeding, or protruding tissue — surgical emergency: keep at rest, cover protruding tissue with saline-moistened gauze, and activate EMS/transfer.

Post-operative hemorrhage — heavy vaginal bleeding with clots or hemodynamic change — escalate; transfuse/transfer per severity.

Pelvic infection / sepsis — fever with purulent drainage and pelvic pain, or systemic signs — escalate on the sepsis pathway.

DVT / PE after pelvic surgery — unilateral calf swelling/pain, or sudden dyspnea/pleuritic pain/hypoxia — escalate/transfer (time-critical).
