Access thrombosis

AV fistula without thrill/bruit — dialysis unit/provider notified urgently, access protected, monitoring, plan updated to preserve access, representative notified; time-critical; supports Section O.

PD peritonitis

Cloudy effluent with abdominal pain and fever — provider/unit notified urgently, effluent studies per order, empiric management coordinated, monitoring, representative notified; reassess; supports Section O.
