AV fistula without thrill/bruit — dialysis unit/provider notified urgently, access protected, monitoring, plan updated to preserve access, representative notified; time-critical; supports Section O.

PD peritonitis

Cloudy effluent with abdominal pain and fever — provider/unit notified urgently, effluent studies per order, empiric management coordinated, monitoring, representative notified; reassess; supports Section O.

Base renal content, AKI, and general fluid-electrolyte management route to the Genitourinary/Renal reference; cardiac fluid overload routes to the Cardiovascular reference and CHF overlay; anticoagulation for access/AF routes to the Anticoagulation overlay; the renal diet routes to the Nutrition reference.
