5. Skilled interventions and teaching

Skilled interventions

• Implement and verify the individualized toileting program and document continence outcomes; protect the skin from moisture.
• Provide catheter care per protocol, maintain a closed system, and re-evaluate the medical necessity daily; remove promptly when no longer indicated.
• Recognize UTI early (including via mental-status change), obtain the ordered studies, and support antibiotic stewardship.

Teaching / training (to mastery)

• Teach the resident/representative the toileting program, catheter care (if applicable), and UTI warning signs, with teach-back.
• Train CNAs on the toileting schedule, perineal/catheter care, and CAUTI prevention, with return-demonstration.
