Urosepsis — fever/hypothermia, hypotension, new confusion, tachypnea with a urinary source — escalate on the sepsis pathway; transfer per severity. Acute retention with distension and no output — urgent catheterization/provider notification (bladder injury risk). Obstructed catheter — no output with distension despite a catheter — troubleshoot/irrigate per order or replace; notify provider. Gross hematuria / clots (especially on an anticoagulant) — notify provider; route to the Anticoagulation overlay. |
