7. Skilled-need justification (SNF)

Skilled procedure

Insertion, sterile irrigation, and replacement of catheters are direct skilled services under Chapter 8 when required.

Observation & assessment; management & evaluation

Assessing UTI/retention/fluid status and managing the continence/catheter plan for an at-risk resident require RN judgment, especially when the presentation is a mental-status change.
