Scenario

Model daily note (edit to the shift)

Neutropenic fever

New fever with a low ANC — oncologic emergency: provider notified urgently, cultures and evaluation obtained, empiric antibiotics per order without delay, close sepsis monitoring, representative notified; reassess; supports Section O/I. (Sepsis overlay.)

Transfusion reaction

New fever/chills with dyspnea and back pain during a transfusion — transfusion stopped, line kept open with saline, vitals obtained, provider/blood bank notified per protocol, unit/tubing retained, monitoring, representative notified; reassess; supports Section O.

Bleeding on thrombocytopenia

New bruising with gum bleeding and a falling platelet count — bleeding precautions, provider notified, platelet support and labs per order, bleeding surveillance, representative notified; reassess; supports Section I.
