Skilled interventions

  • Administer blood products with the identity/consent check and pre-/intra-/post monitoring for a reaction; monitor counts and trends.
  • Provide bleeding-precaution management and bleeding surveillance for thrombocytopenia and infection precautions and fever surveillance for neutropenia.
  • Recognize and immediately manage a transfusion reaction and a neutropenic fever with prompt escalation.

Teaching / training (to mastery)

  • Teach the resident/representative bleeding or infection precautions (as applicable), the warning signs, and the purpose of the transfusion/therapy, with teach-back.
  • Train CNAs on bleeding/infection precautions and reporting fever, bleeding, or a transfusion-reaction sign immediately, with documented understanding.
