Neutropenic fever — any fever in a neutropenic resident — oncologic emergency: urgent provider notification, cultures, and empiric antibiotics without delay; sepsis → transfer. Transfusion reaction — fever/chills, dyspnea, back pain, hypotension, or rash during a transfusion — stop the transfusion, keep the line open with saline, and escalate per protocol. Major bleeding with thrombocytopenia — significant mucosal, GI, or intracranial bleeding with a low platelet count — escalate/transfer. Severe symptomatic anemia — chest pain, marked dyspnea, or hemodynamic change — escalate/transfer. |
