Post-colostomy day 5: stoma beefy-red and viable, soft brown effluent of adequate volume, peristomal skin intact, appliance secure and changed per schedule, self-care teaching ongoing. Skilled stoma assessment, care, and teaching required; supports the Section H ostomy item.
Post-GI-bleed observation
Day 3: no recurrent melena/hematochezia, vitals stable, no orthostasis/pallor, Hgb stable, tolerating the diet. Skilled observation for re-bleeding required daily; supports Section I.
C. difficile with isolation
Contact precautions maintained for active C. difficile, stool frequency/character trending down on therapy, hydration monitored, skin protected. Skilled monitoring and isolation management required; supports Section O/H.
