Recurrent GI bleed

New melena with HR 104 and orthostatic lightheadedness on an anticoagulant — vitals/orthostatics obtained, anticoagulant held per parameters, SBAR, labs/evaluation ordered, representative notified, hemodynamic monitoring; reassess; supports Section I/N. (Anticoag overlay.)

Suspected obstruction

New distension with firmness, absent bowel sounds, vomiting, no BM for days — oral intake held, provider notified, abdominal film/evaluation ordered, abdomen/vitals reassessed, peritoneal-sign monitoring; skilled escalation required.

Stoma ischemia

Stoma now dusky at the base with reduced output — provider/surgeon notified urgently, stoma reassessed and documented, appliance adjusted for visualization, monitoring for progression; event documented.
