Perform the abdominal/GI assessment and, for a new ostomy, stoma and peristomal care with viability assessment.
Provide skilled observation for re-bleeding, obstruction, or dehydration with parameter-based escalation; maintain isolation for an infectious process.
Administer and monitor GI/IV medications and the bowel regimen and document the response (route enteral feeding to Nutrition).
Teaching / training (to mastery)
Teach the resident/representative ostomy self-care, dietary guidance, hydration with diarrhea, and warning signs, with return-demonstration/teach-back.
Train CNAs on ostomy-output and stool reporting, isolation precautions, and prompt reporting of GI change, with documented understanding.
