Recurrent GI bleeding: a new melenic stool with mild tachycardia (HR 104) and lightheadedness on standing, in a resident recovering from an upper-GI bleed and on an anticoagulant — concerning for re-bleeding. Vital signs and orthostatics obtained, the anticoagulant held per parameters pending review, SBAR to the provider, orders for labs and evaluation received, and the responsible party notified; the resident monitored for hemodynamic change. Held oral intake, provider notified, an abdominal film and evaluation ordered, the abdomen and vital signs reassessed, and the resident monitored for peritoneal signs; supports the Section update. Skilled assessment and escalation required.
