Subjective — ask

  • Skin concerns the patient or caregiver reports — breakdown, itching, rash, or skin tears.
  • History of pressure injuries or slow-healing skin.
  • Incontinence and moisture exposure, and the skin-care routine.
  • Mobility and repositioning ability (detail in Function) and the support surface in use.
  • Nutrition and hydration as they affect skin (with Nutrition and Hydration).
