4. Normal findings

The reference range below is the yardstick for “normal/at-baseline.” Normal findings are documented in Mode A with the skilled rationale that follows in Section 6 — never as “WNL” alone.

  • Skin intact with appropriate color, temperature, and moisture.
  • No pressure injury and no non-blanchable erythema at the pressure points.
  • Low pressure-injury risk, or an elevated risk mitigated with prevention in place.
  • No moisture-associated skin damage.
  • No concerning rashes or lesions.
  • Adequate turgor and no abnormal fragility.
  • No existing wound (any wound is routed to the Wounds / Pressure Injury reference).
