8. Interventions performed

Document the skilled interventions delivered this visit; each should connect to a finding and carry a reassessment in Section 10.

• Skin assessment with Braden Scale risk scoring and identification of the subscale drivers.
• Pressure-injury prevention — a repositioning schedule, a pressure-redistribution surface, heel offloading, and skin protection.
• Moisture management — incontinence care and barrier products for moisture-associated damage.
• Skin protection — moisturizing and skin-tear prevention — and coordination for any existing wound (routed to Wounds / Pressure Injury).
• Coordination of nutrition for skin integrity (with Nutrition) and of a support surface or referral as indicated.
