10. Response and reassessment

Every intervention and teaching action requires a documented response that closes the loop. An intervention without a reassessment is an audit gap.

• The effect of prevention — a non-blanchable area resolving with offloading, or skin remaining intact in an at-risk patient.
• Caregiver competence after teaching — a return demonstration of repositioning or skin care.
• Teaching effectiveness: teach-back of prevention and early-recognition.
• Whether the risk is mitigated or what remains to be addressed.
• The plan for reassessment timing and what will be re-evaluated next visit.
